$1000 per day retainer (9am - 5pm standard) plus 20% of production

Travel time to and from the practice is billed at $150 per hour at my discretion.

Overtime is charged at $150 per hour.

For ER, I charge a retainer of $500 per night, and 50% of production if there is more than one ER visit (the first is free)

For ER (weekend), I charge a retainer of $2000 and 50% of production if there are more than 4 ER visits (the first 4 are free).
